Steven L. Kelley, FACHE, President & CEO of Ellenville Regional Hospital (ERH), presented at The New York State Association for Rural Health Conference at Mohonk Mountain House, New Paltz, NY on September 14, 2017, with Susanne Callahan, MA, Assistant Vice President, Planning & Development of The Institute for Family Health (IFH). This presentation described the collaboration between the Ellenville Regional Hospital (ERH), a rural Critical Access Hospital, and The Institute for Family Health (IFH), operator of the Ellenville Family Health Center, focusing on improvements in health outcomes and efficiencies in staffing. It explored how the two facilities work together to address the public health, health care and mental health care needs of the local population. Located on the same campus, ERH and the IFH share medical staff, and have implemented protocols to reduce inappropriate emergency room use, increase access to primary care, and actively address opioid abuse. The ERH emergency department now routinely screens for depression and diabetes control, and refers to The Institute for behavioral health care and management of chronic illnesses. The health center benefits from the hospital’s diagnostic testing and rehabilitation services. In addition, IFH residents now train at both ERH and the Institute’s Ellenville center, where they learn to provide outstanding primary care in medically underserved rural communities. The presentation discussed how ERH and IFH collaborate via monthly strategic and tactical planning meetings, addressing the opioid epidemic, continuity of care, facilities, shared staffing to improve quality, chronic conditions & mental health care, and family medicine residency training.
