ERH in the News-American Hospital Association Leadership Summit
Ellenville Regional Hospital was selected by the American Hospital Association to present two projects, via poster boards, at their 24th annual Health Forum and Leadership Summit in San Diego, CA from July 17-19, 2016. Representing Ellenville Regional Hospital at the summit were Ashima Butler, VP & Chief Quality Officer, Ann Marie Guntlow, Chief Nursing Officer, Theresa Aversano, Director of Physical Medicine and Rehab, and Maria Gonzalez, Manager, Emergency Services.
The first project, New York State Medicaid Accelerated eXchange (MAX) Project, is a 12- month initiative which places a focus on the Emergency Department (ED) providers and staff on managing care for ED Super Utilizers - patients who present to the ED repeatedly seeking opioid treatment for chronic pain management. The parameters of the program include identifying and prioritizing process improvements that will maximize long-term outcomes and clinical efficiency for these patients, and align them with their respective primary care provider and other community resources for their pain management and other healthcare needs.
The second project, the Swing Bed Outcomes Improvement Project has been developed to be able to quantify our Sub-Acute Rehab (Swing Bed) patient’s functional outcomes from admission to discharge. A measurement tool was created utilizing the Barthel Index of Activities of Daily Living. This project involved multiple disciplines: Nursing, Physical Therapy & Occupational Therapy. The goals of the project are to improve the functional independence of swing bed patients; maximize the monthly percentage of swing patients who return home or to their prior level of residence; improve the communication among the rehabilitation team and increase efficiency in working together, and to educate patients families and caregivers to ensure that a safe discharge is established.
