ERH Highlighted under HANYS Triple Aim Campaign
Ellenville Regional Hospital was highlighted this week by the Hospital Association of New York State (HANYS) under their NYS Triple Aim campaign for their work in medication safety.
The hospital was recognized for preventing adverse medication incidents through its innovative and integrated Community Medication Safety Program. This program educates community members, providers, and consumers on medication use, dosage, interactions, and disease management. The hospital expanded clinical pharmacy services through the Community Medication Safety Program by collaborating with the Albany College of Pharmacy, The Institute for Family Health, Matthews Pharmacy, Senior Housing located on the hospital’s campus, and other local practitioners to enable effective hands-off communication with a comprehensive standardized medication reconciliation document.
Thousands of community members have benefitted from this program and adverse medication incidents have been avoided.
Each week, HANYS highlights the many ways their member hospitals and health systems are transforming healthcare, calling the campaign "NYS Triple Aim" because there is a widespread effort to improve population health, enhance the quality of patient care, and reduce the cost of healthcare.
